The Physical and Consent forms can be found on the TSSAA forms page. Both are in the same document. Make sure you turn in the page from the physical on which your doctor has checked that you are "Medically eligible for all sports...."

The Concussion and Sudden Cardiac Arrest forms can be filled out electronically using "Final Forms" (see instructions for creating an account below.)

Final Forms

Sign up for "Final Forms" to submit athletic paperwork online. You can also create an account by scanning the QR code posted on Stephanie's door.

Athletes who played a fall/winter sport: You still need to create an account and fill out all forms electronically. If you have a copy of your physical, please upload it or turn it in to Coach Janis or Stephanie. If you turned in your only copy it can be uploaded for you.

Athletes who have not played a sport yet this school year: Create an account and fill out all forms electronically. Upload the physical form or turn it in to Coach Janis or Stephanie.
